A confirmation email will be sent to the email address you provided. The email will have an attachment (pdf with all 3 forms). You can also download them here below. The Interactive "link" will download all 3 forms together in 1 pdf file and you can use your computer to complete the forms, or you can just print it and complete manually. Please remember to print and then sign each form.
The Application, Waiver/Release, and Medical Self Certification must be completed and signed before you will be allowed on the mats. Please bring all 3 forms (signed) to the first day of class.
